A form that is completed after a review of the incident report, healthcare provider (HCP) reports, as well as parent or staff interview when indicated. Used to document staff, parent or HCP report of a suspected or diagnosed concussion, and to initiate the school concussion protocol with resultant CMT review and development of an initial student support plan (SSP).
